Cost
Calculate the real costs of your health insurance—
more than just bi-weekly or monthly premium costs, be sure to include copayments and out-of-pocket costs for excluded services and out-of-network care that other plans might not cover at all.
Flexible tools to help manage costs—
The Empire Plan offers a number of ways to control health insurance costs including:
- Reasonable copayments
- Basic Medical Program—Plan pays 80% of the reasonable and customary charge after you satisfy an annual deductible if you use a non-participating provider for medical and surgical services. You are reimbursed up to 100% of reasonable and customary charges after you reach the out-of-pocket maximum for your employee group.
- Basic Medical Provider Discount Program offers discounts from certain physicians and providers who are not part of the EP network. These providers are part of the nationwide MultiPlan group and accept a discounted fee for covered services. Basic Medical Program provisions apply and you must meet the annual deductible. Your 20% coinsurance is based on the lower of the discounted fee or the reasonable and customary charge.
